Rules for making an appointment for an initial appointment / consultation / examination

1. General Provisions

1.1. The IEM Clinic provides consultative, diagnostic and inpatient, including high-tech, medical care under the programs of compulsory medical insurance (CHI), voluntary medical insurance (VHI); under direct contracts with enterprises, institutions and organizations; for a fee under contracts for the provision of medical services to individuals.

1.2. In the event that citizens apply for emergency indications and it is impossible to provide appropriate medical care at the current time, the patient is sent to provide the necessary medical care to the appropriate medical facility.

1.3. During the initial visit to the IEM Clinic, you can make an appointment both with a narrow specialist (in the case when the diagnosis is known or the nature of the disease is obvious), and for an examination.

1.4. The patient chooses the time of appointment with the doctor with the participation of the medical registrar in accordance with the appointment schedule and taking into account the availability of free time in the doctor’s appointment schedule.

1.5. In the absence of a doctor, the medical registrar informs the patient about this and provides the opportunity to choose the time to make an appointment with a substitute doctor for the desired date and (or) time.

2. Making an appointment for an initial appointment / consultation / examination at the Clinic of the Federal State Budgetary Scientific Institution ‘IEM’

2.1. The clinic of the Federal State Budgetary Scientific Institution “IEM” provides medical and specialized, including high-tech, medical care in inpatient and outpatient settings at the address: 197198, St. Petersburg, Maly Prospekt of Petrograd Side, 13.

2.2. The operating mode of the Clinic of the Federal State Budgetary Scientific Institution ‘IEM’ is around the clock for inpatients. Outpatient reception is carried out from Monday to Friday from 9.00 to 16.00 in agreement with the doctor.

2.3. You can make an appointment for an initial appointment / consultation / examination with the doctors of the FSBSI ‘IEM’ Clinic as follows:

by calling the receptionist of the FGBNU “IEM” Clinic at (812)235-02-92 on weekdays from 9.00 to 16.00;

through the call-center of a single appointment for appointments with doctors of the medical departments of the Federal State Budgetary Scientific Institution ‘IEM’, by calling (812) 454-69-00;

through a personal appeal to the reception of the Clinic of the Federal State Budgetary Scientific Institution ‘IEM’ on weekdays from 9.00 to 16.00.

3. Information that the patient must provide to the medical registrar when making an appointment for an initial appointment / consultation / examination by phone

3.1. When receiving services under the CHI policy:

Last name, first name, patronymic, date of birth, registration address, contact phone number, MHI policy data.

3.2. When receiving services under the VHI policy:

Last name, first name, patronymic, date of birth, registration address, contact phone number, VHI policy details, name of the insurance company.

3.3. When receiving services under a contract for the provision of medical services for individuals:

Last name, first name, patronymic, date of birth, registration address, contact phone number.

4. Appearance for the initial appointment / consultation / examination

4.1. On the day of the first visit for a consultation (examination), you must arrive at the Clinic 15 minutes before the appointed time to issue a medical card.

4.2. To issue a medical card, the patient must provide the following documents to the medical registrar:

– upon receipt of medical services under the compulsory medical insurance policy: passport, compulsory medical insurance policy, SNILS, referral for a consultation with a doctor of the basic outpatient clinic specified in the Annex to the compulsory medical insurance policy;

– when receiving medical services under the VHI policy: passport, VHI policy;

– when receiving medical services for cash: passport.

4.3. If the patient is late for more than 5 minutes from the appointed time, the reception staff and doctors have the right to refuse the patient a scheduled appointment (consultations, medical diagnostic procedures, etc.) and reschedule the provision of medical services to the late patient for another time.

5. Documents issued based on the results of admission / consultation / examination

5.1. Upon completion of the appointment / consultation / examination, the patient receives the following documents:

– Therapist’s opinion (if necessary).

– The results of diagnostic studies with decoding.

Doctor’s recommendations for further treatment or health maintenance, containing a scheme of outpatient drug treatment.

In case of incapacity for work, a certificate of temporary incapacity for work (sick leave) is issued, which is drawn up in the prescribed manner.

Referral to inpatient treatment (if necessary).

A set of payment documents (when providing paid medical services).

IT IS IMPORTANT:

The patient’s medical record is the property of the Federal State Budgetary Scientific Institution ‘IEM’, is stored in the Clinic of the Federal State Budgetary Scientific Institution ‘IEM’ and is not issued to patients.

It is not allowed to take out a medical card from the FSBSI ‘IEM’ Clinic without the consent of the FSBSI ‘IEM’ Administration.

The issuance of a medical card in hand is carried out with the permission of the head physician of the Clinic of the Federal State Budgetary Scientific Institution ‘IEM’ on the basis of a written application from the patient after registration in a special journal.

The medical card is issued under a personal signature, personally in the hands of the patient. The issuance of a medical card to relatives is prohibited.
